Trying to replace my old sim


I recently returned from the US and I hadn't used my sim card since August 2016. I went in to get a replacement for my iPhone and the service representative helped me out but my phone is still not connected. I received an email today saying ''Unfortunately we are unable to process your SIM Activation request as the information you provided us with does not match what we have on our system.'' My reference number provided was 04219710
