Sim activation

Hi, 

I lost my phone the day before Christmas eve and I've just went and got a replacement sim today. After filling out the required questions to best I could my request came back failed. I haven't rang anyone in the last 7 days as asked because I use whatsapp 99% of the time. Can anyone from Three support help activate my new sim, I can provided numbers from my call log.

Have you logged into My3 to see how your account was registered? What you put in for name, address and birthday? Copy that on the form.
