Proof of usage & blacklisted

I requested for proof of usage & blaclisted by calling you guys on 26/03/2018 when I lost my phone. After 6 calls to your customer service i managed to get a letter that my phone had been blacklisted but I'm waiting on proof of usage to be able to make an insurance claim. When I did this I was told that it would be up to 48 hours before one was sent out.

I waited until now, can you please tell me when can i get it



Can you confirm how long it takes and that a request has actually been sent? At the moment, this form from Three is the only thing holding up my claim.
